UPON RECEIVING A REFERRAL, RIVER CREST WILL:

Request any additional information.

Verify insurance benefits.

Obtain an accepting physician.

Provide notification of decision ASAP.
PRIOR TO TRANSFER OF PATIENT:
Patient meets criteria

Patient does not meet
criteria for admission: |
• Must have been seen by a
physician the day of admission
(within 24 hours)
• Physician must recommend patient
needs treatment at River Crest
Hospital
• Memorandum of Transfer must be
completed and faxed or sent with
patient to River Crest.

• River Crest Hospital will provide appropriate referrals:
• Out-patient treatment,
• Long-term treatment,
• State funded treatment facilities,
• Other referrals as needed.
