  Referring a Voluntary Patient to River Crest

       From a Hospital

  • Call Intake Department at 325-949-5722 or 800-777-5722
  • Provide patient's name, date of birth, address, phone number, social security number and insurance information.

  • Provide description of symptoms or diagnoses, and nature of current crisis.

        UPON RECEIVING A REFERRAL, RIVER CREST WILL:

  • Request any additional information.

  • Verify insurance benefits.

  • Obtain an accepting physician.

  • Provide notification of decision ASAP.

 

                         PRIOR TO TRANSFER OF PATIENT:

Patient meets criteria

for admission:

                                          Patient does not meet

                                         criteria for admission:

• Must have been seen by a 

  physician the day of admission

  (within 24 hours)

• Physician must recommend patient

  needs treatment at River Crest

  Hospital

• Memorandum of Transfer must be 

  completed and faxed or sent with

  patient to River Crest.

• River Crest Hospital will provide appropriate referrals:

    • Out-patient treatment,

    • Long-term treatment,

    • State funded treatment facilities,

    • Other referrals as needed.

  • Provide patient's name, date of birth, address, phone number, social security number and insurance information.

  • Provide description of symptoms or diagnoses, and nature of current crisis.

               UPON RECEIVING A REFERRAL, RIVER CREST WILL:

• Request any additional information.

• Schedule a free assessment.

• Provide information or referrals as needed.

                           

                               THE ASSESSMENT PROCESS:

Patient meets criteria

for admission:

                                    Patient does not meet

                                   criteria for admission:

• Clinical Interview
• Physical examination

• Psychosocial assessment

• Therapeutic and Chemical use

  evaluation

• Admittance onto unit

• River Crest Hospital will provide appropriate referrals:

    • Out-patient treatment,

    • Long-term treatment,

    • State funded treatment facilities,

    • Other referrals as needed.
